REASON® US Defence Force Deployment

The REASON® deployment in the US Defence Force includes US Naval Air Command (Test Wing and Safety), US Naval Academy, the US Naval Air Warfare Centre, the Office of Naval Intelligence, US Navy Trident Refit Facility, Kingsbay Nuclear Submarine Base, and Norfolk Naval Shipyards

 

In terms of Certification, REASON® has received NMCI Certification for installation and use on the Navy and Marine Corps shared global IT infrastructure.

 

Confidentiality requirements limit our comments on this REASON® deployment to saying it is used for investigation of ordinance handling, flight-testing, safety and maintenance issues.  Including continuous improvement of practice standards, accident and incident investigations.

 

US Space Program and NASA Deployment.

The United Space Alliance have an extensive REASON® deployment. They are the prime contractor for NASA’s Space Shuttle Program and under the Space Flight Operations Contract with NASA, performs space shuttle launch and landing operations at the Kennedy Space Center in Florida and flight operations for the Johnson Space Center in Houston. 

 

The United Space Alliance REASON® deployment is designed to aid in future emergency responses, problem solve, address prevention and safety issues, and for engineering planning within United Space Alliance’s projects.

US Home Land Security

Home Land Security REASON® deployments include the National Emergency Events team that investigate energy accidents, including nuclear incidents.  REASON® was used by the Department of Energy; Federal Electricity Regulatory Commission and North America Electric Reliability Council for reporting to the US Government on the August 2003 America and Canada Electricity Blackout.

 

Aircraft Accident Investigations

In relation to aircraft accident investigations, REASON® has been used successfully in military and commercial environments.  Non-Defence related deployments of REASON® for air safety investigations have less stringent confidentiality requirements. 

 

A Non-Defence related analysis is an International Society of AirSafety Investigators Presentation given by Mr Jean-Pierre Dagon, Director Corporate Safety for AirTran Airways. 

 Mr Dagon’s presentation uses an example where Aircraft 123, Flt 455's left wing collided with a parked fuel truck.  CLICK HERE 

 

This presentation provides a detailed overview of how environmental, engineering and human factor elements of an investigation are combined into one logical tree model that identifies the root causes and the percentage each root cause contributed to the resulting incident.
